·        Before participating in our Residential Health Education Program or being seen at our outpatient clinic, we will need to have your Personal Information and Medical History forms completed.   

      Note: You will see a confirmation page which means we have successfully received your information and you may proceed to the next step.

·        For your convenience, we have provided the following links in  STEPS 1-3 to allow you to complete your forms prior to your visit.

·        Once you have completed the admission forms  please call us at (707) 586-5555 so that we can answer your questions, schedule an out-patient office visit, or schedule a stay at our residential health education program.
