Before participating in our Residential Health Education Program or being seen at our outpatient clinic, we will need to have your Personal Information and Medical History forms completed. This is done via our secure patient portal.

Step 2. Within 24 hours, we will email you a link to our patient portal and login information.
Step 3. Follow the instruction on the patient portal to complete the requisite forms.
