Before participating in our residential health education program or being seen at our outpatient clinic, we will need your Personal Information and Medical History Forms completed. This is done via our secure patient portal. Here's how to get started:
NOTE: Please be sure to include your email address on the registration form so we can send you a link to our patient portal.
Step 1: Click the link below to access and complete the registration form.
Step 2: After a staff member processes your form (generally within 24 hours), you will receive an email with a link to our secure patient portal. Please click the link and complete the medical history forms.
Step 3: After you have completed and submitted the forms, you will receive a confirmation email with follow-up questions. Once you have replied to this email with your answers to the questions, we will arrange for you to have a no-cost phone screening with Dr. Alan Goldhamer.
For our foreign friends: Our system cannot handle foreign postal codes at this time. Please add your postal code to the street address line and use our zip code (95404) to complete registration. We will update the code after you complete our medical health questionnaire.
